IS this a lost cause? I mean Joomla has an inherent problem dealing with duplicate URLs.

Yes, management of this problem is doable..using robots.txt, nofollow/noindex using htaccess for redirects.. its manageable. I think I see it differently than most because the hoops I have to jump through. Not many would go to the lengths I have to avoid duplicate urls.

Is this really an issue?

It's not that easy to solve and unfortunately not done in 5 minutes. :(

As we do this in our free time, I can just say it is done when it's done. Sorry about it and that it is causing trouble for you, but that's how it is.
